Activities Team Registration

Sports Physicals/MSHSAA Medical Eligibility Form
Registration will be complete once your child's MSHSAA Medical Eligibility Form has been uploaded to this site.  Sports physicals are valid for 12 months from the date of the exam.

 

Participation Fee
No participation fee will be charged during this registration process.  When you get to Payment Options, please select Option 2 - Allow parents to choose to Pay Later. 
NFL tickets may be picked up at the school office to help offset the participation fee of $275.
